Hazard Analysis- Analysis Form Settings page lets you change the settings of your Hazard Analysis form.
You can navigate to this page by selecting any form in the Analysis list or clicking Add New.
This page has the following items (as labeled above):
1. Analysis Information
This section lets you change the most basic information about the analysis. It has the following items:
Analysis Name
The name of your analysis.
Analysis Form
The name of the form your analysis is going to use. You can create analysis forms from the Forms page.

2. Analysis Configuration
This section lets you change the settings of you analysis forms. It has the following items:
Allow adding Crew after Form Submission
The checkbox, when enabled, lets a user add crew after an analysis form has been submitted. This might be useful when there is a new crew member that also helps with the hazard analysis.
Restrict the form user to be registered
The checkbox, when enabled, lets you allow only registered members to fill and submit the form. This feature might be helpful when you need detailed information about the users that fill the form. It could also be used to register any recurring users that haven’t registered yet.
Enable Signature
The checkbox, when enabled, lets you add a signature for the form submitter in the hazard analysis form. Enable the Make it Mandatory checkbox to make it mandatory for the form submitter to sign before submitting the form.
Acknowledgement Text
The acknowledgement text box lets you add text into it that will be displayed to the form submitter before they submit the form. The form submitter must read the text and check the box above it to ensure that they have read the text before submitting the form.
Enable Crew Signature
The box, when enabled, lets you add signature boxes for the crew in the form. Enable the Make it Mandatory checkbox to make signing mandatory for the crew members of an analysis. You can also add an acknowledgement text for the crew which they have to read and check before the submitter submits the form.
